Puberphonia,We have treated 544 cases up to this year end (31.12.2019).Today I need a new level of thinking, leadership and strategic competencies to envision a better future and devise innovative mechanism to reshape it & sustain it.

VOICE IS IMPORTANT BOSS
Conversation is one of the most basic forms of communication, but don’t underestimate it. A simple, conversation with your clients can build trust. An innocuous bit of small talk with a stranger can turn into a sales opportunity. Learn to speak in an approachable voice, confident way that you can apply to any situation.
 The sound of our voice makes a strong impression. Here are the most important elements to keep in mind. Resonance:-Resonance is defined as “the quality in a sound of being deep and full most important of all, every voice projects the personality of its speaker. In other words, voice determines personality. We use our voice; it should have the force and frequency for people to listen.    Possibly, the raised voice used to listen others and modulation with adequate frequency helps others to understand our views.
A speech disorder is a condition in which a person has problems creating or forming the speech sounds needed to communicate with others. Some speech disorders can cause people to develop anxiety disorders. Stressful situations can trigger anxiety, resulting in more pronounced speech disorder symptoms.
Pathophysiology
There are three basic types of speech impairments: articulation disorders, fluency disorders, and voice disorders.
The most common speech disorders in adults
Stuttering: It may come as a surprise that many famous actors and entertainers experienced stuttering, including James Earl Jones.
Apraxia: If you have apraxia, your tongue and lips aren't able to move in the correct way to produce sounds.
Dysarthria:  Voice disorders involving the larynx (voice box). Dysarthria can occur when you have trouble moving the muscles of your lips, tongue, vocal folds, or diaphragm. It can result from degenerative muscle and motor conditions, such as multiple sclerosis, muscular dystrophy, cerebral palsy, Parkinson's disease or stroke.
Aphasia:  It is a communication disorder that results from damage or injury to language parts of the brain. It's more common in older adults, particularly those who have had a stroke. Aphasia gets in the way of a person's ability to use or understand.
Cluttering: It is a speech and communication disorder characterized by a rapid rate of speech, erratic rhythm, and poor syntax or grammar, making speech difficult to understand words. Aphasia does not impair the person's intelligence.
Dysprosody:  which may manifest as pseudo-foreign accent syndrome, refers to a disorder in which one or more of the prosodic functions are either compromised or eliminated completely. Prosody refers to the variations in melody, intonation, pauses, stresses, intensity, vocal quality, and accents of speech.
Verbigeration: It is obsessive repetition of random words. It is similar to perseveration, in which a person repeats words in response to a stimulus. However, verbigeration occurs when a person repeats words without a stimulus.
Tangential speech:  Tangentiality is a communication disorder in which the train of thought of the speaker wanders and shows a lack of focus, never returning to the initial topic of the conversation.
What is poverty of speech? In psychology, alogia (Greek ἀ-, “without”, and λόγος, “speech”), or poverty of speech, is a general lack of additional, unprompted content seen in normal speech. As a symptom, it is commonly seen in patients suffering from schizophrenia and schizotypal personality disorder, and is considered a negative symptom.
What is clanging speech? In psychology and psychiatry, clanging refers to a mode of speech characterized by association of words based upon sound rather than concepts. For example, this may include compulsive rhyming or alliteration without apparent logical connection between words.
Speech anxiety: Can range from a slight feeling of “nerves” to a nearly incapacitating fear. Some of the most common symptoms of speech anxiety are: shaking, sweating, butterflies in the stomach, dry mouth and rapid heartbeat. Pressured speech is when someone's speech is rapid and disorganized.
Pressured speech/rapid speech: It is not a disease or disorder by itself. Rather, it is a symptom of an underlying disorder. There are many potential causes of pressured speech, including schizophrenia, some forms of anxiety, and bipolar disorderly voice.
Puberphonia:  Puberphonia is a disorder predominant of post-pubescent male (without known organic cause), who exhibit higher register than other male of the same age group, known as "Juvenile Resonance Disorder" or “Little Girls’ Voice”. The high voice may be produced at the top of the chest register or in falsetto; this high voice may be associated with mild dysphonia and increased, effortful phonation. Vocal instability is often marked with extensive frequency swing like cutting of voice while speaking continuously. Vocal impairment in male is generally of higher pitch i.e. above 200Hz, and sometimes observable with downward pitch breaks. Downward pitch break reveals the natural lower pitch level of 110-125 Hz.  Possibly elevated than the average airflow, if breathy voice accompanies higher pitch.
There are a number of proposed causes for the development of this speech problem. The etiology of puberphonia can be both organic and psychogenic in nature. Causes of puberphonia include: 1. Emotional stress 2.Delayed development of secondary sex characteristics.  3. Resistance to pubertal changes 4. Self-consciousness resulting from an early breaking of the voice
More importantly, they are not coming out or converse or mingle with other person or society; because of they feel ashamed of their voice. After ADEM.S APLE formation they supposed to talk “BOSS VOICE rather talking with high frequency like female folk. As a result they cannot explain their view or even a truth of an occasion of explaining the real cause for any issues which could make them too ashamed of the situation. So they psychologically affected with depression. Besides, they cannot retain in their job longer period. The frequent quitting the job which leads to financial constrain in their family. Negative social reactions commonly occur and it overall affects social and vocational goals of them.  Actually they suffer in their all walks of their life, like physically, psychologically and economically.
The habitual use of a high pitch while speaking is associated with tense muscles surrounding the vocal folds. Assessment and treatment of puberphonia is usually conducted by an otolaryngologist (ENT). This voice is not a disorder that is likely to go away on its own. Without treatment, the changes in the patient's voice can become permanent. Treatment can involve direct voice therapy, indirect voice therapy, or audiovisual feedback. In most cases when traditional voice therapy is ineffective. RESEARCH DESIGN
        The study was conducted in Siva E N T Hospital at Chennai. So far we corrected 515 puberphonic clients.
   The study concludes that the Voice care management in SIVE E N T Hospital resolves all the negative impact of the puberphonic clients and provides renaissance in their life in all directions.
